MTP does not arrange emergency or non-emergency transportation by ambulance.
To get a ride through MTP, you can’t have any other means of transportation. You must also be enrolled in one of the following programs:
Before you call us, set up an appointment with your doctor or provider.
Call us at least two work days before you need a ride locally or within your service area.
If you need to travel out of town or out of your service area, call us at least five work days before you need a ride. If you have a health or dental plan and need to travel out of your service area, call those plan providers too so they can document your travel.
Children 14 and younger cannot travel without a parent or guardian. Children 15 through 17 can travel without a parent, but the parent must fill out a consent form before the trip is scheduled.
Call us just like you would to schedule a ride. Tell the customer-service representative you have a car but need help with gas money. They will mail you an application form to become an Individual Transportation Participant.
Medicaid can also pay someone else to drive you to your appointments, like a relative, friend or neighbor, through the ITP program.
You or your driver must have a current driver’s license, license plates, up-to-date inspection sticker and car insurance. Each trip must be approved by MTP before you go to your scheduled appointment.
