  • Patients are seen by appointment only and must meet eligibility requirements. All services are available in English and Spanish. If you have questions about services or eligibility, please call 817-632-6020. The clinic is located at 3500 Noble Avenue  Fort Worth, Texas 76111. We serve people who are 18 years or older, without health insurance, and with an income not greater than 250% of National Poverty Level. Se habla español. 

    Criteria for Eligibility / Criterio de elegibilidad:

    • 18 or older / 18 años o más
    • Do not have health insurance* / No tiene seguro de salud
    • Income not greater than 250% of National Poverty Level / Ingresos inferiores al 250% del nivel nacional de pobreza
    *We do not accept or bill insurance. If you have Market Place coverage, you must provide proof of your high co-pay and inability to pay. Donations are not accepted in return for clinic services.
