Ascend Nurse Family Partnership - Approved

• A program for first-time mothers. Involves registered nurses making in-home visits throughout pregnancy and continuing to visit until the baby is 2 years old. Mother has to be under 28 weeks pregnant. The program also provides car seat training and can provide car seats to clients in need. Clients enrolled can also receive diapers/wipes assistance. There is currently no waitlist.

  • The Nurse-Family Partnership is a free program that partners first-time moms with experienced nurse home visitors. When you enroll in the program, a specially trained registered nurse will visit you throughout your pregnancy and until your baby turns two years old. During these visits, your caring nurse will offer the knowledge and support you need to confidently create a better life for you and your baby.

     

    Qualifications: 

    -Pregnant with your first child

    -Meet income guidelines

    -Be 28 weeks or less pregnant. You can register as early in your pregnancy as you like. We recommend that your nurse home visits start by your 16th week of pregnancy.

    - Financial Education: classes as well as 1 x 1 (offers a wide range of help regarding finances)
