    Have a medical diagnosis of a life-threatening medical condition (does not have to be terminal).

    Be between ages 3 and 19 years old when the form is received. 

    Have not previously received a wish from any wish-granting organization.

    Reside in or are being treated in Texas.

    *Apply on website. Once application is submitted, a request will be sent to physician to sign and confirm child has life-threatening condition.
