Initial Nutrition Assessment/Consultation
Consist of a one-on-one, 2-hour session with Elizabeth. This service includes a detailed assessment of your child’s growth, biochemical, clinical, dietary and feeding history. Elizabeth will review your child’s current lab tests results, food record, supplements and feeding issues; prescribe a customized vitamin/mineral supplement; recommend diet modifications and special diets. This service takes place via phone. After the session is completed you will receive a written report of your child’s individualized Nutrition Assessment/Nutrition Care Plan.
This service begins with you completing a Nutrition Questionnaire and forwarding to Elizabeth along with any prior laboratory tests results and additional information on your child that you consider important. Elizabeth will review prior to your initial session (So you do not lose time on the actual phone call!).
Follow-up Consultation

follow-up sessions are recommended every 3 months to update your child’s Nutrition Care Plan adjusting for growth, dietary changes, and implement additional nutritional interventions. This service takes place via phone.
