A rapid rehousing program to move participants from homeless, to housed, to independence. Participants must have the ability to work or gain income in a few months and move towards self-sufficiency. Our goal is to assist people for 6-9 months after they are housed with a rental subsidy, attaching to long-term supports (behavioral health, employment, treatment, etc.), then graduate them to paying their full rent.
Appropriate candidates will be those who are willing and able to work (not waiting to receive Social Security benefits as the sole source of income) and motivated to change.
Eligibility
• Homeless
• Disability diagnosis (mental health diagnosis and/or substance dependency diagnosis)
Referrals
People wanting a referral should go to the Day Resource Center (DRC) at 1100 E. Lancaster Ave., Fort Worth to complete the prioritization process or speak to their shelter case manager to complete a referral. Diagnosis can be obtained at DRC as well. Referrals should be submitted to Evyn.Ellis@mhmrtc.org.
Documents Needed:
· Homeless documentation (A letter from the nighttime shelter the person is residing and a copy of bed stays.)
· Proof of mental health or substance dependency diagnosis (copy of diagnosis and/or letter from licensed professional)
· Proof of income
· Proof of assets
· Social Security card
· Picture ID
· Birth Certificate
