    Cost:

    As a Federally Qualified Health Center, we provide care to anyone, regardless of income, residency status, employment, or Health Insurance coverage. If you are uninsured, fees for service will be based on a sliding fee scale determined by your household income and family size. To determine eligibility, you will need to bring in either a Tax Return or proof of income for one month and proof of dependents. If you qualify you could receive up to an 85% discount.

    Hope Clinics services are not free. We request payment in the form of copay at the time of visit. If you are unable to pay for services at the time of visit a payment plan is offered with a 20% down payment. Hope Clinic accepts payment in the form of cash, check, credit card or money order. If you need to mail in your payment you can do so at: 411 E. Jefferson, Waxahachie, TX 75165.
