• Detailed Information

  • The PrimaCare Program is a partnership between Collin County Health Care Services and PrimaCare Medical Centers. This county funded program is an option for program eligible families who earn less than 100% of the federal poverty guidelines and who are in need of primary care or urgent care services in our community. If eligible for the program, the patient will pay a $20.00 co-pay and the county will pay the difference of the cost of the visit. This program is available to Collin County residents of all ages. Family-friendly office hours, a variety of services and locations throughout the county are just some of the options available to patients who qualify. Application forms are provided at the locations listed below.     Eligibility: To determine if someone is eligible to receive medical assistance through this program, the household must earn less than 100% of the federal poverty guidelines. Applications are completed at the clinic and a current valid state ID is required to be presented at the time of visit.  Please see the URL under Links & Resources section for federal poverty level information.

    ·       PrimaCare: Offers primary care, urgent care, and occupational medicine services seven days a week. No appointment is necessary. There are 5 locations throughout Collin County to serve you. Please reference the PrimaCare website to find the location that will best serve you.

    Many locations are offering telehealth or curbside care. They do Covid 19 testing.  They take many types of insurance including medicaid.
