For those with no health insurance who do not qualify for any other programs or assistance, CHSA offers a sliding fee schedule based on Federal Poverty Level (FPL) guidelines for family size and income. The health center staff members can help you decide on the payment plan most appropriate for the client.

Insurance, Medicaid, CHIP, Medicare (where applicable), Medicaid-on-hold for
patients 2 years and under, Cash, and Uninsured assistance available based on
family size and income (a sliding fee).
This clinic operates under a sliding scale model. This means that services may not be free depending on your income. The client will be required to prove financial need in order to receive free services or
services at a reduced cost. This is a health care center funded by the federal
government. This means even if you have no insurance you can be covered. The
center is also income based for those making an income. This health center can
cover services such as checkups, treatment, pregnancy care (where applicable),
immunizations and child care (where applicable), prescription medicine and
