Visitor policy: One healthy adult will be allowed per patient inside our clinic, per visit. Please designate the primary caregiver who will go into the room with your child upon arrival. Siblings and other visitors will not be allowed.
Mask policy: Parents and families, as we continue to follow the recommendations from the CDC, if you have an appointment at our primary care offices, please bring and wear your own mask or cloth face covering from home.

Scheduling through MyCookChildren's Patient Portal is currently limited to only telemedicine visit options.
Well child visits for any child are now considered essential and will be scheduled according to availability.
All attempts will be made to schedule well-child and sick visits separately.
Exam rooms will be thoroughly cleaned after each visit with extra cleaning provided to common spaces.
Call between 7:30 a.m. – 5 p.m., Monday – Friday and between 9 a.m. and 11 a.m. on Saturdays. We are better able to answer your questions and give you advice when we can have your child's medical records on hand.
A nurse will take your call, or if one is unavailable, leave a message with a receptionist and a nurse will return your call. Rapid responses are very important to us.
Routine office visits are by appointment only; we will make every effort to accommodate your needs when your child is ill.
Please call as soon as possible if you must cancel or reschedule your appointment.
Our office is closed most holidays, including New Year's Day, July Fourth, Thanksgiving and Christmas.
