    Eligibility Criteria:

    ·       Residence:  Must live in Erath County and intend to stay in Erath County

    ·       Income:  May not exceed the minimum of 21% of the Federal Poverty Income Level

    ·       Resources:  May not exceed $2,000 per month (or $3,000 if aged or disabled)

    ·       Citizenship:  US Citizen

    ·       Medical Need:  Must have a medical need of some kind

    ·       Application:  Packet must be completed in entirety and all documentation provided

    ·       Applicant:  Must show proof of SDI/SSI or Medicaid denial

    Erath County is the payor of last resort
