Community Health Clinic provides
basic healthcare to uninsured children and adults who fall below the 200%
Federal Poverty Level in northern Collin County (excluding Plano and Wylie). We
offer a wide range of programs and services to meet the needs of the medically
- Primary Care
- Diabetic Care
- Women Wellness Exams
- Pediatric Care
- School Physicals
- Prescription Assistance Programs (only for their patients)
- Physical Therapy
You will need to call on Tuesdays between 9am to 12pm to qualify for services.
Patient Must bring the following documentation to their registration appointment. They program can be contacted on their website.
Current picture ID with current address.
Current copy of
utility bill, rent/mortgage statement or car/house insurance with name and
Proof of household income.
This may be:
Most current pay stubs for one month
W-2 or most recent tax return
Proof of child support
Social Security Benefits
Commissions, fees & tips
Government funded housing
If you are paid cash, you must bring a statement from your employer stating wages and a contact number.
