PAF’s Co-Pay Relief (CPR) program provides direct financial assistance to qualified patients with co-payments, co- insurance or cost-sharing associated with prescription drugs through funds dedicated to specific disease states. In some instances, assistance with insurance premiums and/or ancillary services associated with the disease also may be available.
Services
include:
- Financial assistance for co-payments, co-insurance, and deductibles
- Financial assistance with prescription medications.
Patients must be currently insured and have coverage for the medication(s) for which they seek financial assistance. (Including federal plans such as Medicare).
Patient must have a physician confirmed diagnosis of the disease under which they are requesting financial assistance.
Patient must reside and receive treatment in the United States.
Patient's income must fall below the income guidelines. Income brackets vary between 300% and 400% or less of the Federal Poverty Guideline with consideration for the Cost of Living Index and the number in the household.
