Medicaid or CSHCN clients receiving a Medicaid covered service and who have no other way to travel to their appointment, may request transportation services from LogistiCare. LogistiCare takes request for routine transportation Monday through Friday from 8 a.m. to 5:00 p.m. Please submit requests for routine transportation 48 hours (2 business days) in advance of the trip. Requests for Advanced Funds should be submitted at least 5 business days in advance of the trip.
Information that is needed a ride is requested is:
You need your Medicaid ID or CSHCN number. You will need the name, address, zip code and telephone number of your medical provider. You will need the street address and the telephone number where you want to be picked up. You will also need the date and time of your appointment, any special needs you have and the general reason for the appointment (doctor's visit, check-up, eye appointment, etc.).
