To get a ride through MTP, you can’t have any other means of transportation. You must also be enrolled in one of the following programs
· Medicaid
· Children with Special Health Care Needs
· Transportation for Indigent Cancer Patients
Call MTP at least two work days before you need a ride locally or within your service area.
If you need to travel out of town or out of your service area, call MTP at least five work days before you need a ride. If you have a health or dental plan and need to travel out of your service area, call those plan providers too so they can document your travel.
Children 14 and younger cannot travel without a parent or guardian. Children 15 through 17 can travel without a parent, but the parent must fill out a consent form before the trip is scheduled.
When you call, you’ll need to:
1. Have you or your child’s Medicaid ID, Children with Special Health Care Needs Services Program number or Social Security number.
2. Give the address where you will need to be picked up. If there is a phone number at the place that you are being picked up from, please provide it.
3. Give the name, address and phone number of the doctor, dentist office, drug store or place you plan to go. MTP will also ask you for the date, time and reason for your visit.
Tell MTP if you or your children have any special needs, like a wheelchair or a walker, so they can send the right type of vehicle
