qualifying patients with non-local medically-related transportation when
traveling more than 50 miles but not exceeding 300 mile trips, to assist with
reaching medical care.
Includes:
- Gas cards (Provided to help off-set fuel cost for patients)
- Bus Tickets (Greyhound, Trailways, etc.)
transports clients to medical care over 300 miles away, but does not require medical care during the flight. Patients must be ambulatory.
: Proof of Income: First page of your most recent Federal tax return, social security, and Medicaid or disability statement and verification of your medical
appointment (i.e. a letter from your physician, a printed appointment reminder, or a
screen shot of your patient portal showing the appointment date).
