Perinatal Connections/Family Connects North Texas - West - Approved

• A program under Perinatal Connections, offers a free in-home visit by a Registered Nurse to postpartum clients/ families up to 12 weeks post-delivery in Tarrant, Parker, Hood, Johnson, Wise, and Erath counties. A Registered Nurse will provide maternal and infant physical assessments, including blood pressure check for mom and a full head-to-toe assessment for infant (including weight & measurements), postpartum depression screening, maternal and infant warning signs, breast and formula feeding support, and infant care education. The Registered Nurse will also complete a community needs assessment and will link families to community resources, as needed.

• Detailed Information

  • Residents of North Texas can receive a free in-home nurse visit when their child is about 3 weeks of age up to 12 wks. Family Connects nurses can help address families' needs such as baby and mom health check, postpartum depression screening, breast or bottle feeding help, safe sleep information, playgroups and parent support groups, back-to-work support, and any additional support needed by the family.  

    In-home visits are typically done when baby is 3 weeks of age, but baby must be less than 12 weeks to receive services.

    They are associated with affiliate hospitals in Tarrant, Wise, Johnson, Hood, Erath and Parker counties including Lake Granbury Medical Center, THR Cleburne, THR Harris, Medical City Arlington, Arlington Memorial or Wise Health.  Families not delivering in those hospitals can self-refer themselves for services.

    At least one home visit is provided, but up to 3 home-visits if needed.
