• Detailed Information

  • Anyone can make a referral including the parent. There is no actual physical location as they work in the home, school, and community.  

    Services:  

    ·         Crisis Intervention:

    o   The crisis, the perception of an unmanageable situation;

    o   The individual or group in crisis; and the case manager who provides aid.

    o   Crisis intervention requires that the person experiencing crisis receive timely and skillful support to help cope with his/her situation before future physical or emotional deterioration occurs

    ·         Medication Management and Training:

    o   Learn about the mental health diagnosis.

    o   Have support to reduce symptoms related to the mental health disorder.

    o   Learn about medications and how they can help reduce symptoms related to the mental illness

    ·         Skills and Training Development:

    o   Functional skills and appropriate behavior related to the individual’s health and safety

    o   Instrumental activities of daily living, and use of community

    o   Monitoring and education regarding mental health, nutrition, and physical condition

    o   Use of community resources

    o   Organization and Routine

    o   Anger management skills

    o   Enhancement of social and interpersonal skill

    ·         Targeted Case Management:

    o   The TCM process is a shared partnership between the client and/or family and community supporters and the case manager.

    o   Client and/or family, community supporters are actively involved in all phases of the process – assessment, planning, problem solving and identification of resources.

    o   TCM ensures available resources are efficiently accessed and being used
